Flu shot will be given on November 15, 2011 in RSB 01 Foundation Conference Room from 11:00am-1:00pm
-Vaccination is $25/participant unless covered by insurance (must present proof of insurance/not all plans cover flu vaccine)(we will have a list of plans that cover it)
-Consent forms will be available at the time of clinic
-We can vaccinate ages 4 and over (patients under 18 require parent/guardian consent)
-We have preservative-free vaccine available if anyone has an allergy to thimerosal (a mercury-containing preservative)
